Support Services

There is a range of support services available to assist the person living with dementia, their carer and family. As your needs change, the support you require will also change. 

 

It can feel like a maze, trying to find the right service at the time you need it.  It is hoped that the list of services described on this website will assist you. 

 

When accessing services it can at times be difficult to know where to start, or who to contact, if you are having difficulty you can contact an Access and Support worker.

 


Access and Support workers can assist people living with dementia and their carers / families, who are experiencing difficulties finding the right services for them.

 


Access and Support workers can provide:
•Support to person living with dementia and their carer / family free of charge
•Connection between you and service providers - supporting you to discuss your needs and access the services that are right for you
•Access and Support workers do not provide services, but rather link you to them
•At the client's request the Access and Support worker can be present at assessments and the care planning meetings
•Access and Support workers focus on the person, their strengths, needs and preferences, along wih those of the carer and family

 


Access and Support workers are not case managers and can work with people experiencing difficulties in finding the right service for up to 12 weeks, connecting you to the services right for you.
